Not for emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room. Examples include chest pain, trouble breathing, signs of stroke, severe bleeding, severe allergic reactions, or thoughts of harming yourself or others. If you're in crisis, you can call or text 988.
Where you must be during your visit
You must be physically located in Ohio at the time of your Y-City Telemedicine visit. If you're in Florida, please book through Tropical Telemedicine instead.
Website information is not medical advice
Content on this website is general information. It isn't a diagnosis or treatment plan, and it doesn't create a provider-patient relationship. That relationship begins only after you book, complete intake, and have a visit with our provider. We can't give medical advice through the contact form, email, or social media.
What telehealth can and can't do
Many common conditions can be treated by video, but some can't. During your visit, our provider may decide that you need an in-person exam, testing, or emergency care. Prescriptions are written only when our provider decides they are medically appropriate. Booking a visit does not guarantee a prescription or a specific treatment.
Medications
Prescriptions are sent to a pharmacy. Some weight-loss medications and wellness therapies we may prescribe are made by compounding pharmacies. Compounded medications are not FDA-approved. They are prepared for an individual patient based on a prescription. Your provider will review the benefits, risks, and alternatives with you.
Results
Every patient is different. We don't guarantee any specific outcome, including any amount of weight loss.
Payment
Visits and programs are self-pay and are paid by credit card at booking. We don't bill insurance. We can provide visit notes and an itemized receipt on request so you can seek reimbursement on your own.
Refunds are handled case by case. If you need to cancel or have a question about a charge, call or text 941-376-8630.
Consent
You'll review and sign our telehealth consent and other forms during intake. Those forms are part of these terms.
Using this website
Please don't misuse the site, try to access parts of it you aren't authorized to use, or copy our content for commercial use. Links to outside websites are for convenience. We aren't responsible for their content.
Limits of liability
The website is provided as is. To the extent the law allows, we aren't liable for damages from using or being unable to use the website. This doesn't limit any responsibility we have for the medical care we provide.
Governing law
These terms are governed by Ohio law.
